An Excellent Opportunity
Mountain Valley Hospice is seeking a Registered Nurse (RN) Hospice Liaison to serve on-site at Atrium Health Wake Forest Baptist Medical Center in Winston-Salem.
In this role, you will serve as the connection between the hospital and hospice—working closely with physicians, case managers, patients, and families to ensure a smooth, timely transition to hospice care when appropriate.
You will respond to hospice referrals, provide education, complete assessments, and facilitate admissions—often meeting patients and families during critical decision-making moments.
What This Role Looks Like
- Maintain a consistent hospice presence within the hospital
- Partner with physicians, case managers, and hospital staff to support patient care planning
- Educate patients and families on hospice services and the end-of-life care journey
- Complete initial nursing assessments and facilitate hospice admissions
- Coordinate the referral-to-admission process, including documentation and care planning
- Perform hospice-in-place (hospital) admissions when appropriate
This role requires strong clinical judgment, the ability to communicate clearly and compassionately, and comfort supporting patients and families facing serious illness and end-of-life decisions.

- Monday – Friday
- 8:00 AM – 5:00 PM
Qualifications
- Active Registered Nurse (RN) license
- BLS certification
- Valid driver’s license
- Experience in hospice, palliative care, or acute care preferred
- Strong communication skills and ability to work collaboratively in a hospital setting
