Registered Nurse - PRN
RESPONSIBILITIES:
This position requires the flexibility to work any of the seven days of the week, helping ensure uninterrupted, compassionate, care 24 hours a day, 7 days a week. A responsible member of the IDG providing skilled nursing services to TCC patients on an as needed basis. Responsible for identifying patient/family needs and providing supportive care and appropriate services in accordance with physician’s orders and the IDG Plan of Care.
- Assumes primary responsibilities for a patient/family that includes the assessing, planning, implementing, and evaluating phases of the nursing process.
- Conducts admission assessments as assigned and incorporate data into the development of the initial Plan of Care.
- Conducts skilled nursing assessments and reassessments per Plan of Care.
- Perform specific nursing procedures and services according to agency policies and procedures.
- Communicates in a timely and accurate manner significant changes in patient condition to the attending physician and IDG.
- Participates in IDG and facilitates discussion of pertinent issues surrounding the patient/family unit and Plan of Care.
- Provides documented supervision and direction to CNA every 2 weeks.
- Evaluates patient/family level of knowledge and understanding of Plan of Care.
- Formulates teaching plan based on knowledge deficits.
- Informs Care Team of unusual or potentially problematic patient/family issues.
- Maintains up to date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
- Provides appropriate support at the time of death according to Policies & Procedures.
- Demonstrates familiarity with agency Policies & Procedures and federal and state regulations.
- Adheres to organizational Policies & Procedures on infection control, standard/universal precautions and use of PPE.
- Maintains confidentiality according to Policies & Procedures.
- Demonstrates the ability to communicate effectively with patient/families, attending physician and other IDG members.
- Maintains professional relationships with patient/caregivers.
- Continues education through workshops, in-services and self-directed learning.
- Demonstrates competency per annual RN competency assessment.
- Demonstrates dependability and reliability in job performance.
- Participates in agency and community programs as requested to promote professional growth and understanding of Hospice care, including performance improvement activities.
- Shares in providing 24 hour a day/7 day per week on-call coverage to patient families as scheduled.
- Performs other duties as may be assigned by the Director of Nursing.
QUALIFICATIONS:
- Valid, NC, RN license in good standing.
- Available for night, weekend, and holiday on-call duty.
- Preferred Demonstrated understanding and support of the philosophy and objectives of Hospice.
- Preferred two-year nursing experience with current knowledge of nursing practice in an acute care setting.
