JOB SUMMARY

Hospice of Southern Kentucky utilizes a nurse practitioner, directed by the Medical Director, to promote palliative care services. Their respect in the medical community will have a positive effect on the utilization of medical care for all palliative care and/or hospice patients. The NP will be responsible for providing patient visits for palliative care patients both new and established and coordinating their plan of care with other designated staff. The NP will be required to attend routine scheduled care plan meetings.

ROLE

As a palliative care nurse practitioner, your responsibilities include ensuring that a patient is as comfortable as possible, working with the family to answer questions and address concerns. Educating the patient and family in helping them with pain and symptom management.

RESPONSIBILITIES/DUTIES

1. Assessment of disease processes and hospice-appropriateness

2. Evaluate effectiveness of current palliative treatments

3. Educate staff, patient and family regarding End-of-Life care

4. Notify MD of changes needed in palliative treatment plan

5. Assist to maintain accurate medical records

6. Communicates clearly

7. Able to travel

8. Other duties as assigned

5. Referring patients to hospice, when deemed appropriate and eligible

REQUIREMENTS (of Position)

1. Current nurse practitioner license

RELATIONSHIPS

This position requires compassion and caring for the patients and families to develop trust in him/her. He/she will work with the Medical Director, nurses, IDT and other employees, as needed.

He/she will most likely have a primary role in the healthcare community, and can have a great impact regarding the utilization of hospice services for the patients/families they contact. This position will also, have the opportunity to educate co-workers in their primary job regarding the benefits of palliative care.

PHYSICAL DEMANDS and WORKING CONDITIONS

Requires sitting for long periods of time; occasional bending, stretching or lifting, and full range of body motion. This position requires the ability to work under high stress conditions for long periods of time. Requires corrected vision, ability to hear normal range of voice, drive, and negotiate steps, walkways or housing not designed for handicap access. May require contact with individuals with communicable disease and body fluids. Occasionally, requires maneuvering items weighing over 100 pounds. Able to work in normal office environment, use office equipment, and prepare written reports.
