What You’ll Do
- Deliver exceptional, compassionate care to hospice patients within our residence.
- Supervise LPNs and Home Health Aides, ensuring quality care and optimal staffing.
- Collaborate with the interdisciplinary team to manage symptoms and pain effectively.
- Perform accurate assessments and develop individualized care plans.
- Administer medications, maintain IV access devices, and operate clinical equipment.
- Serve as a resource and leader for your shift, assisting staff with clinical and operational concerns.
- Ensure documentation meets compliance standards and reflects accurate clinical care.
- Coordinate care with physicians, facilities, and families while upholding the hospice philosophy.
- Graduate of an accredited School of Nursing; current NYS RN license required.
- Minimum 2 years of medical/surgical experience preferred.
- Hospice or palliative care certification a plus (but not required).
- Strong clinical assessment skills, leadership ability, and a commitment to compassionate care.
