Responsibilities include EMR data entry, coordinating admissions, ordering equipment, communicating with care teams and referral sources, educating patients and families on hospice services, and supporting admission tracking and reporting. The position is primarily office-based with flexibility for remote work based on organizational needs.

About This Role
The Referral Coordinator manages hospice referrals from first contact through admission, ensuring accurate documentation, effective communication, and a smooth transition into hospice care. Responsibilities include EMR data entry, coordinating admissions, ordering equipment, communicating with care teams and referral sources, educating patients and families on hospice services, and supporting admission tracking and reporting. The position is primarily office-based with flexibility for remote work based on organizational needs.
What You’ll Do
- Serve as the first point of contact for hospice referrals via phone and e-fax
- Manage the referral process from initial inquiry through hospice admission
- Gather, verify, and enter referral and patient information into the EMR
- Coordinate and schedule hospice admission and informational visits
- Order and document durable medical equipment for new admissions
- Communicate referral details and updates to clinical staff, vendors, and internal teams
- Educate patients and families on hospice philosophy, services, and care expectations
- Provide anticipatory guidance to support patients and families during the admission process
- Track eligibility, admissions, and previously discharged or revoked patients
- Prepare and distribute daily referral and potential admission reports
- Assist with interdisciplinary team meeting preparation
- Ensure compliance with agency policies, procedures, and documentation standards
Education Required:
- High School diploma or equivalent.
- Must be able to excel at communication skills.
- Must be able to work within an environment of critical needs.
- Must be able to demonstrate a high level of organization, multi-tasking skills., and be able to work at a high pace.
- Familiarity with various office equipment, including but not limited to: computers, computer system programs, etc.
- Computer literacy.
- Ability to work independently.
- Associate's degree in Human Services or a related field.
- Two (2) years of experience in a hospital or hospice setting.
- Experience in a health care setting.
