What We Are Looking For
Job Title : Licensed Practical Nurse
Schedule : Saturday - Sunday 8 : 30am - 4 : 30pm
Location : 800 Stony Brook Court Newburgh, NY 12550
Salary Range : $30.00 per hour.
This position is an exciting opportunity to provide care to our patients located in their homes in the Orange and Sullivan County NY area. This position works as a member of the hospice interdisciplinary team to provide support and to implement the Plan of Care initiated by the RN.
Essential Job Functions / Responsibilities
- Provides direct patient care as defined in State Nurse Practice Act.
- Implements current nursing practice following an approved Plan of Care initiated by the Registered Nurse.
- Provide accurate and timely documentation of patient services to reflect the Plan of Care.
- Provide patient / family / caregiver education and information pertinent to diagnosis and individualized Plan of Care.
- Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members, e.g., hospice aide, OT, PT, MSW, ST, and Dietitian.
- Uses equipment and supplies effectively and efficiently.
- Provides appropriate pain / symptom management. Evaluates patient's response to treatments / medications.
- Performs other duties as assigned by the RN.
- Adheres to Infection Control and other pertinent Hospice policies regarding patient family care.
- Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain / symptom management changes / outcomes as appropriate.
- Communicate with primary RN regarding patient's needs and reports changes in patient's condition.
- Teaches the patient / family / caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as directed by RN. Recognizes and utilizes opportunities for health counseling with patients / families / caregivers. Works in connection with the other members of hospice interdisciplinary team.
- Provides and maintains a safe environment for the patient.
- Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Fulfills the obligation of shift assignment.
- Participates in Performance Improvement Program.
