Shift: 8:30 am - 4:30 pm

About This Role
As a Hospice Aide Specialist, you'll make a meaningful difference in the lives of our patients and their families. This role provides direct, hands-on care in alignment with the patient’s plan of care while supporting their comfort, dignity, and well-being. You’ll work closely with our nursing staff and the interdisciplinary team to ensure each patient receives exceptional, personalized support.
What You’ll Do
- Assist patients with daily personal care needs including bathing, grooming, dressing, toileting, and mobility.
- Monitor and report patient observations and changes in condition to the RN.
- Help maintain a clean, safe, and healthy environment.
- Follow proper infection-control and safety procedures.
- Offer companionship through conversation, activities, and emotional support.
- Provide simple nursing support, vital sign checks, and emergency assistance as needed.
- Support caregivers by offering respite and responding promptly to patient/family needs.
- Work collaboratively as part of the hospice interdisciplinary care team.
Requirements
Required Skills and Abilities:
- Valid NYS Home Health Aide (HHA) certification
- High School diploma or equivalent
- Minimum of one (1) year of experience in a hospital, nursing home, or similar clinical setting.
- Ability to read, understand, and follow written instructions, and accurately document care provided.
- Demonstrated initiative and the ability to work effectively with minimal direct supervision.
- Genuine empathy and sensitivity to the needs of individuals who are ill, frail, or functionally impaired.
- Understanding of the philosophy of hospice care and the unique needs of the terminally ill.
- Consistently demonstrate tact, patience, professionalism, and appropriate personal hygiene.
- Strong interpersonal and communication skills.
- Ability to remain composed and resilient in high-pressure or emotionally challenging situations.
- Excellent organizational and time-management skills.
