What is a RN Case Manager?
The RN Case Manager is essential to the Hospice organization. From admission to the end of our patient’s journeys, the RN Case Manager provides professional nursing care by utilizing all elements of the nursing process. The position is our field team leader in our local community. The RN Case Manager leads a designated team that includes a LPN and Hospice Aide Specialist who are assigned to their own personal territory located throughout Orange and Sullivan Counties. In addition to providing hands on medical care, the RN Case Manager collaborates with the Social Work and Spiritual Care departments to develop a comprehensive plan of care.
