Hospice of Orange and Sullivan Counties is seeking a full-time Nurse Practitioner to join our team. As a member of the interdisciplinary team, the Nurse Practitioner will provide meaningful, end-of-life care where it matters most—at home, in nursing facilities, and at our Kaplan Family Hospice Residence.

Responsibilities May Include
- Conducting in-home patient visits for symptom management, collaboration of care, goals of care discussion, assessment of complex wounds, and evaluation of eligibility for hospice benefits.
- In collaboration with the medical director and intake nursing staff, reviewing clinical documents to determine eligibility and terminal diagnosis.
- Serving as the agency’s on-call provider, which includes advising nursing staff on symptom management, prescribing medications including controlled substances, and completing death certificates.
- Serving as a clinical expert to nursing and other staff.
- Participating in community education and outreach.
Education, Skills & Experience:
- MS Nurse Practitioner or DNP Doctorate Nurse Practitioner with NYS license and national certification.
- Must hold a current NY State RN license.
- DEA license to prescribe controlled substances in New York State.
- Current State of New York driver’s license and automobile insurance.
- Two (2) years of Nurse Practitioner experience, preferably in primary care or home health.
- Strong preference will be given to candidates with hospice experience and certification.
