Qualification Requirements
- Must be a State Tested Nursing Assistant (STNA) or able to obtain within first 90 days of employment.
- Has working experience charting in an EMR system, Netsmart system preferred.
- High school diploma or equivalent required
- Demonstrates strong computer skills with a working knowledge of Microsoft Office applications - Word, Excel and Outlook. Must have excellent data entry skills.
- One year experience with scheduling coordination; previous experience in homecare preferred.
- Possesses strong verbal and written communication skills.
- Possesses excellent organizational skills and the ability to schedule work in order to meet deadlines.
- Possesses a valid driver's license with an automobile in good working order that is insured according to agency requirements.
- Is committed to the philosophy, goals, and objectives of Hospice of Northwest Ohio.
- Initiates monthly staff schedules for Hospice Aides and other schedules in Homecare as needed.
- Assigns a Hospice Aide to each patient referred for services according to individual patient need, geographical location and Hospice Aide availability, within established agency guidelines.
- Communicates with families and caregivers to coordinate the referral process.
- Contacts/notifies the assigned Hospice Aide, patient, family, ECF charge nurse and primary nurse/team leader to set up a schedule for HHA visits and substitutes on a day-to-day basis. Informs individual HHA of the daily cancellations and refusal of care or any other reason patient may not want or need a HHA visit on that day.
- Documents the assignment, occurrences or communication with patient/family in the assignment on a progress note. Alerts primary nurse/team leader of any special needs or concerns.
- Responsible to learn and utilize agency technology made available by the agency to increase efficiency, communication and performance of duties.
- Attends staff meetings and completes all mandatory training.
- Will provide homecare visits, if necessary.
- Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
- Performs other duties as assigned.
