General Description: Responsible for being the initial point of contact representing Hospice of Montezuma and facilitating communication and necessary paperwork between patients, families, physician offices, and Hospice of Montezuma staff.

Level of Responsibility: Direct supervision from the Operations Director.

Duties and Responsibilities:

  • Responsible for first line communication regarding patient referrals.
  • Maintain a positive work environment by acting and communicating with internal and external contacts in a manner that actively promotes teamwork, problem resolution, and customer satisfaction.
  • Track status of all referrals through the admission process and follow up with whoever responsible for next steps to complete the process.
  • Prepare necessary forms for staff, families and physicians.
  • Track and follow up on forms and paperwork sent and received.
  • Enter information into Electronic Medical Record (EMR) accurately and timely throughout the process. Keep updated. Review for accuracy.
  • Follow up any missing paperwork or forms.
  • Prepare forms, and packets of forms, for completion. Referral and admission forms. Assemble admission packets and consult packets
  • Deliver or pick up paperwork as necessary.
  • Review forms being sent to physicians and families to make sure they are professional and appropriate in appearance.
  • Facilitates communication between all staff regarding status of referrals
  • Track status of failed referrals and discharged patients for future follow up and staff contact needed.
  • Provide backup to administrative staff in answering phone calls and greeting guests.
  • Assist with fundraising, publicity, and public relations as requested.
  • Represent Hospice of Montezuma with professionalism in public.
  • Other duties as requested by the Executive Director.

Experience: Two years of office experience preferred.

Qualifications:

  • Ability to maintain confidentiality in all matters related to patients and staff.
  • Ability to present a calm and professional demeanor at all times. Professionalism, courtesy, and compassion are critical.
  • Excellent computer skills and willingness to learn specialized computer programs.
  • Demonstrated ability to work with diverse team.
  • Ability to assess, prioritize, and complete work according to deadlines.
  • Willingness to dedicate the time necessary to accomplish assigned tasks.
  • Must take initiative in following up multiple tasks
  • Ability to keep track of many things simultaneously in an organized manner
  • Problem solving skills. Able to negotiate new situations and apply knowledge
  • Ability to delegate, track and follow up multitude of items.
