RN (Full-time, Hospice Admissions) Job Summary:
Hospice of Lenawee, a 5-STAR Medicare.gov rated hospice, seeks a compassionate Registered Nurse for a full-time, hospice admissions position. The Hospice Admissions RN will meet with patients and families, explaining the hospice benefit, and facilitating their transfer onto hospice services. Hospice experience preferred but we are willing to train. This role is a calling for someone dedicated to patient-centered care at the end-of-life journey.

RN (Full-time, Hospice Admissions) Schedule:
- Full-time (40 hours per week/5 days per week)
- Day shift
- Weekend and Holiday rotation
RN (Full-time, Hospice Admissions) Requirements:
- Graduate of approved school of nursing
- Current Michigan RN License
- BSN, preferred
- Nursing experience, required
- Experience with dying patients and their families, hospice experience preferred
- Experience in health care, required
RN (Full-time, Hospice Admissions) Duties & Responsibilities:
- Develops a comprehensive care plan that addresses physical, emotional, sociological, and spiritual needs of the patient and family in conjunction with the patient, family, and program staff.
- Teaches, supervises and counsels the patient and family regarding care issues.
- Maintains accurate, complete, and timely documentation on patient's record.
- Encourages timely consideration by hospital staff and physicians for hospice as an alternative mode of care, assisting in identification of appropriate patients for the purpose of supportive care for terminally ill patients.
- Develops and maintains relationships with area Physicians, Nursing Homes, Hospitals, etc. and their staffs.
- Familiarity with community health organizations, practices and resources.
- Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.
