Licensed Social Worker - PRN

Cornerstone Hospice & Palliative Care of Georgia is seeking a per diem Licensed Social Worker (LMSW, LCSW) to assist, educate and provides support to patients and families with financial, social, and emotional concerns as well as provide bereavement services. 

 SCHEDULE: On Call as needed; Sun-Mon: 4:30pm-8:00am 

 JOB DUTIES 

•  Provides social work services in accordance with the patient's plan of care. 
• Completes the social services section of the admission process including, but not limited to, the psychosocial assessment, patient and family education about hospice benefits, and the gathering of financial information. 
• Attends and participates in interdisciplinary team meetings. 
• In collaboration with the interdisciplinary team, the patient, and the patient's family, develops an appropriate plan of care that addresses personal, financial, and environmental issues impacting the family. 
• Assists the interdisciplinary team in understanding the significance of social and emotional factors related to patient care. 
• Maintains individualized patient care documentation with accuracy, timeliness, and according to policies. 
• Assesses and reassesses social, emotional, and financial factors in order to minimize problems related to the patient's life-limiting illness. 
• Identifies and utilizes community resources to assist with patient and family needs. 
• Provides crisis intervention for patients and their families, when and where appropriate. 
• Keeps abreast of hospice social services trends and knowledge. 
• When requested, provides education and training for Cornerstone Hospice, participates in in­ service programs, and participates in Cornerstone Hospice-sponsored events. 
• Provides clinical supervision and training when needed. 
• Participates in On-Call rotation. 
• Oversees the provision of bereavement services in accordance with applicable laws. 
• Other duties as assigned. 

QUALIFICATIONS 

1. Master's degree from a school of social work accredited by the Council on Social Work Education. 
2. Licensed Master Social Worker or Licensed Clinical Social Worker required.
3. One year of experience in medical social work or a related field, or equivalent hospice internship is preferred. 
4. Able to demonstrate knowledge, skills, and commitment to the hospice philosophy of care and the hospice team concept. 
5. Able to assess data reflecting the patient's status and circumstances, and to plan interventions accordingly. 
6. Applicable driver license and required auto liability insurance.
