Description:
We are seeking a Palliative Care Navigator, with a focus on principal care management. This is a full-time (40 hour) position, Monday through Friday. The palliative care navigator is responsible for enrolling patients in the care management program, developing individualized care plans, conducting follow-ups to monitor progress and address needs, and overseeing patient engagement and program participation to ensure continuity of care and achievement of health goals.
Requirements:- Active Certified Medical Assistant (CMA) certification or willing to obtain.
- Experience in a clinical office setting, palliative & hospice care preferred.
- Computer skills are required.
