Position Title: Full Time Social WorkerDUTIES:Social work evaluation of patient/family psych/social and environmental needs.Implements the social work treatment plan and documents consistent with insurance guidelines in their homes or facilitiesAssists the primary nurse with establishing and maintaining a treatment plan for each patient/family.Provides the psychosocial needs of the hospice patient/family.Assesses the needs of the bereaved client through verbal and face-to-face interviews.Provides end-of-life bereavement supportive services for patients and their families.Understands and discusses available community resources with the client for possible referrals.Participates in community education on grief and bereavement issues.Provides support to selected patients post discharge.Represents the philosophy and services of Hospice of Huntington, Inc.POSITION QUALIFICATIONS:MSW or BSW with appropriate proof of licensure/certification.Minimum 1 year experience in a health care settingLicensed in WV and OhioDemonstrate evidence of knowledge in understanding grief, bereavement, interpersonal issues and family dynamicsValid driver's license along with an insured, reliable vehicle required.Minimum of one year experience in health care, with a preference in Hospice/Home Health Care.Evidence of clinical experience and skills pertinent to assessing and counseling individuals
