Overview
The Care Coordinator provides clerical and administrative support to the Palliative Care program. This position answers, screens and directs phone calls, including coordinating the process of new referrals, schedules patient appointments and works closely with patients, family members/caregivers and Palliative Care staff to ensure efficient coordination of services.
Key Responsibilities
- Answer, screen, and direct phone calls related to palliative care services.
- Respond to inquiries from patients, families, and providers; explain program services and eligibility.
- Coordinate the process of new referrals to the palliative care program.
- Schedule patient appointments and manage calendar logistics.
- Collaborate with providers, nurses, social workers, and other staff to ensure smooth care coordination.
- Enter and maintain accurate patient and insurance information in electronic health records.
- Communicate documentation needs for billing and insurance compliance.
- Provide administrative support, prepare reports, and assist with outreach activities.
Required Education
- High school diploma or equivalent; additional education in healthcare or related field preferred.
Required Skills and Abilities
- Strong communication and interpersonal skills.
- Ability to work collaboratively with patients, families, and healthcare professionals.
- Proficiency in using phone systems and computer applications, include electronic medical record systems.
- Strong organizational skills and attention to detail.
Preferred Experience
- Experience in a healthcare setting, preferably in palliative or hospice care.
- Experience in scheduling and coordinating patient appointments.
