Hospice Social Worker Position
We are seeking a full-time Hospice Social Worker to join our team. This position offers:

Summary:
The Hospice Social Worker is responsible for assisting patients and families in managing the psychosocial, emotional and resource needs related to end of life issues.
This is a Monday-Friday position from 8:00 a.m. until 5:00 p.m., with an on-call requirement.
Recent graduates are encouraged to apply.
This position requires a Master's degree in social work from a Council on Social Work Education accredited school, current Michigan Social Work licensure as a clinical social worker, and a valid Michigan driver's license.
Responsibilities:
- Completing admission and transfer processes with patients and families;
- Providing counseling and education on psychological, emotional and social aspects of death and dying;
- Assisting patients and families in identifying and utilizing resources;
- Documenting patient/family contact and activity.
Qualifications:
- Master's degree in social work from a Council on Social Work Education accredited school;
- Current Michigan Social Work licensure as a clinical social worker;
- Valid Michigan driver's license;
- Sound clinical judgment and ability to work independently;
- Knowledge of and/or experience with end of life care;
- Ability to communicate effectively.
