Job Summary:
Responsible for providing nursing services to terminally ill patients in their home setting and supportive services to family members in compliance with all applicable federal, state & local health and safety codes including LSC(R), laws and regulations, and all applicable policies and procedures of Hospice of East Texas.
Job Qualifications:
- Current Texas RN license
- Documentation of at least two years or equivalent of nursing experience in an acute care setting preferred
- Current CPR Certification as required by policy
- Hospice experience preferred
- Valid Drivers License
- Proof of Current Automobile Liability Insurance
- Reliable transportation
- Demonstrate ability to relate effectively with physicians, health care professionals, the community, patients/families, and members of the hospice interdisciplinary team
- Proficient with Microsoft Office software relevant to position; ability to master software applications relevant to position (including but not limited to the electronic medical record)
- Ability to read, write and comprehend English
- Employee must be FIT tested for an N95 mask
- Essential Functions & Responsibilities:
- Knowledge of local, state, and federal laws for delivery of hospice services
- Demonstrate specific communication skills, and the ability to interact with fellow employees\Has the ability to perform the nursing process of assessment, planning, and implementation and evaluation.
- Apply specific criteria for admission and re-certification for hospice care to establish appropriate levels of care and patient’s eligibility for hospice services
- Implement the individualized plan of care and recommendations for revisions to the plan as necessary
- Ensure nursing care complies with patient’s plan of care and is documented appropriately ·
- Coordinate all patient/family services and prioritize the needs with members of the IDT/IDG
- Provide initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.
- Assess risk for pathological grief
- Assess cultural and spiritual implications of end of life issues
- Maintain awareness of verbal and non-verbal patterns of communication of patients/families
- Manage discomfort and provide symptom relief utilizing specialized nursing skills related to palliative and end of life care
- Educate the patient and family regarding the disease process, self-care techniques, issues related to end of life care, and the processes for dealing with issues of ethical concern
- Initiate appropriate preventive and rehabilitative nursing procedures
- Determine the scope and frequency of services needed based on acuity and patient/family needs
- Demonstrate knowledge of medication administration, interactions and side-effects
- Demonstrate knowledge and competence in pain and symptom control concepts, and ensure interventions are carried out on a timely basis
- Ensure the case management approach for referral to other services as needed
- Prepare and submit clinical and progress notes that demonstrate progress toward established goals according to policy and procedure guidelines
- Communicate information using current processes/available technology on a timely basis
- Act as physician agent when calling in prescriptions for patients by following agency protocols
- Supervise LVNs, HHAs and paraprofessionals providing services to the patient according to regulatory guidelines; participate in evaluation and corrective action when necessary
- Provide specialized hospice training to other staff, family members and informal caregivers to ensure adequate patient care
- Ensure on-going evaluation of the patient/family response to care
- Assess the ability of the caregiver to meet the patient’s immediate needs upon admission and through out care provision
- Evaluate own needs for support and identify systems to meet the needs
- Assume responsibility for reordering patient medications /maintaining inventory of supplies
- Demonstrate knowledge and awareness of cost containment by economical use of supplies, medications and other resources
- Ensure patient privacy when providing care and treatment
- Participate in clinical on-call as scheduled
- Ensure compliance with Death Pronouncements as per agency protocol
- Complete audits of discharged patient charts within 30 days of discharge
- Participate in the QA/PI process
- Monitor self, staff and patient safety in care setting
- Participate in departmental and other meetings as requested
- Participate in orientation of new staff, volunteers, and training of nursing students as requested
- Participate in all required In-Service Programs
- Maintain current level of skills and evidence of competency
- Adhere to all Confidentiality policies and procedures and maintain confidentiality of administrative information
- Personal growth and development by having flexibility in other work assignments as needed.
- RN act as preceptor for new nurses
- Seeks to increase knowledge and skills through education, in-services, workshop, and certification (CHPN) and maintains credentialing
- Perform other reasonable duties as assigned
- Physical Requirements:
- Frequently required to stand, sit, walk, drive, reach with hand and arms, stoop, kneel, twist, and crouch.
- Regularly required to use hands to handle and feel; and to speak and hear.
- Intermittently lift and/or move up to 50 pounds.
- Ability to deal effectively with stress
- Occasionally required to do standing/sitting presentations involving speaking and listening to an audience.
- Specific vision abilities required by this job include close, distance, color, peripheral, depth perception, ability to focus and ability to drive during varied weather conditions and day/night lighting
