Primary Function:

The Certified Nursing Assistant performs various patient care activities and related nonprofessional services necessary in caring for the personal needs and comfort of patients under the direction and guidance of professional nursing personnel.

 

Training:

Orientation to Hospice, Home Health, Inpatient and Palliative Care services and any other training that may be necessary to complete the assigned duties.

                                       

Qualifications:

          1. High school diploma preferred.

          2. Completion of certificate program in nursing care from an accredited training program with                       current listing as NA-I with North Carolina Department of Health and Human Services                    Regulation. 

          3. Six months to one year experience preferred.

          4. On-the-job training to become familiar with Hospice policies, procedures, and routines.

          5. Ability to work with the team concept of patient/family care outside a hospital setting.

          6. Ability to work interdependently.

          7. Commitment to the Hospice concept and standards.

           8. Must be physically capable of providing patient care in various institutional settings to

               include supporting, moving, and lifting adult patients, and lifting equipment and supplies. 

           9. Must be current in health immunization requirements of North Carolina and Hospice of

               Davidson County.

         10. Must be able to interact with patients, family members, visitors, co-workers, and medical

               staff under a variety of circumstances.

         11. Regular, reliable attendance is essential.

       

 Job Duties: 

The following are the specific responsibilities of the Certified Nursing Assistant:

          1.  Adheres to written and oral instructions for patient care per the nurse.

          2.  Takes and records vital signs, temperature, pulse (radial & apical), respiration, blood                              pressure, weight (stand up scales/bed scales) and height as directed.

          3.  Bathes (assist bed, bath, tub shower, sitz), dresses and undresses patients and assists with

               mouth care, skin care, hair care and nail care to increase their comfort.  Provides lotion and

                massages for patients.  Assists the patient with all aspects of personal care, ambulation                           and positioning as required.

           4. Provide post mortem care when required.

           5.  Assists with bed pans, urinals, and empties and clean ostomy bags.

          6.  Makes beds and changes linens on occupied and unoccupied beds. Assists and maintains

               general cleanliness, sanitation, infection control and isolation technique.

          7.  May be required to prepare meals and assist with feeding.

          8.  Collects specimens as needed.

          9. Body mechanics assisting patient in turning and positioning, range of motion exercising,                      and walking; positions bed to aid in comfort of patient. Use of lifts and assist with ambulation              side rails/call rails etc. Transfer patients from chair and stretcher.

         10. Records intake and output and totals as necessary.

         11. Contributes to the maintenance of nursing care plan.

         12. Reports all unusual conditions or reactions to the patient’s primary nurse and/or Director of

               Hinkle Hospice House.

         13. Completes documentation of patient care and treatment at the end of each visit.

         14. Helps keep supplies organized. Places charge slips in patient notebook when removing                           supplies.

         15. Attends weekly team conference, meetings and educational programs as necessary.

         16. Maintains a cooperative and collaborative relationship with all Hospice personnel that                    facilitates a positive practice environment and interdisciplinary philosophy of services to                 families.

         17. Understands and adheres to the mission and advocates its philosophy.

         18. Follows all policies and procedures.

         19. Maintains confidentiality of Protected Health Information (PHI) at all times.

         20. Acknowledges importance of team awareness.

         21. Addresses patients, co-workers, and other healthcare professionals in a pleasant and

               respectful manner.

         22. Conveys instructions, directions, and information to co-workers.

         23. Consults with immediate supervisor as issues and concerns arise while maintaining                   consistent and concise lines of authority.

         24. Performs housekeeping services that are necessary for the health and safety of the patient.

         25. Cleans toys in play room each shift and as needed.

         26. Follows the guidelines for universal precautions when providing patient care.

         27. Assists in answering the telephone and identifies faculty and self. Directs call to appropriate 

               person or assists caller as needed. Takes messages, documents the date, time, name of     

               caller, phone number and message when applicable.

         28. Answers patients call light timely.

         29. Adheres to all policies, procedures and regulations regarding patient care, conduct, safety,                      fire, security and risk management.

         30. Consistently represents Hospice of Davidson County to all internal and external customers                   with integrity and professionalism.

         31. Attends required in-services on an annual basis:

               A. Risk Management    

              B. Bloodborne Pathogens/TB

               C. Body Mechanics      

              D. Fire Safety

               E. General Safety/Hazardous Materials

              F. Patient/Staff Relations

         32. Maintains a safe and clean work area to promote work place safety.

         33. Clocks in/out on computer.

         34. Is aware of basic pain management including use of pain rating scales.  Recognizes and

           communicates with the Hospice nurse the need for intervention regarding pain when                    indicated.

         35. Documents services and expenditures for reimbursement purposes.

         36. Performs other duties or special projects as requested by the Director of Hinkle Hospice

           House.

While performing the duties of this job, the employee is regularly required to stand, walk, sit, use hand to finger, handle or feel objects, tools or controls; and reach with hands and arms, to talk or hear.

 

The employee is occasionally required to stand, walk, climb, or balance; stoop, kneel, crouch, or crawl and smell.  The employee must occasionally lift and/or move up to 50 pounds.  Specific vision abilities required by this job include close, distance, color, peripheral and depth perception and the ability to adjust focus.

                             

          The noise level in the work environment is usually moderate.
