Job Description
The Scheduler is responsible for scheduling LPN and Home Health Aide visits to Hospice patients. Duties include but are not limited to the processing of patient information, various forms, and all forms of communication to members of the interdisciplinary team.

Qualifications and Skills – Scheduler
The Scheduler must have: -the ability to coordinate multiple ongoing activities and special projects; – the ability to communicate and interpret patient records and standards; -the ability to learn and use computer equipment and various types of software; -must possess strong verbal communication skills, and the ability to interact positively with a variety of customers by telephone. The Hospice Scheduler must possess a basic working knowledge of clinical techniques and comprehension of medical equipment. Must possess a valid NY State Driver’s License and reliable transportation.
Education: High School Diploma required
Experience: No prior experience is necessarily required, but two years of experience within a healthcare related field handling patient records and information is preferred.
Responsibilities – Scheduler:
- Communicates with team members as needed to convey patient
- Accurately inputs patient data into Electronic Medical Records software in a timely
- Schedules LPN/HHA cases as requested, notifies staff and enters information into
- Coordinates with Patient Care Managers and Triage when LPN is needed and communicates availability to necessary staff.
- Runs death report daily and notifies staff of deaths – ensures deceased patient has been removed from scheduler on each HHA/LPNs schedule.
- Enters activity logs into EMR and generates additional reports utilizing EMR as needed for staff
- Audits charts as needed
- Manages patient information and records per agency standards
Job Type: Monday to Friday
Salary Range: $19.75 to $21.00 per hour
- Human services, hospice or related field: 2 years (Preferred)
