Job Description – Registered Nurse Case Manager (RN)J
Hospice of CNY has an urgent need for Nurse Case Managers to coordinate care for terminally ill patients in the final stages of their lives. The RN Case Manager provides for all of their patient’s medical needs while helping to maintain the highest quality of life and comfort. Hospice RNs work with patients in a variety of settings, including in the patient’s home, in residential settings, such as skilled nursing facilities, enhanced care assisted living programs, and in group homes. Hospice RN Case Managers are compassionate, caring people who help guide the patients and families through the end-of-life transition.
Our RNs primarily work Monday-Friday from 8:00-4:30 or 8:30-5:00 and rotate coverage for weekends – typically one time every 6-7 weeks. If you are seeking to improve your work-life balance, with minimal after hour or weekend coverage, becoming a Hospice RN will meet that need!

Qualifications and Skills – Registered Nurse / RN:
Education: Must be a graduate from an accredited school of nursing. BSN degrees are preferred but not required.
Experience: Minimum of one (1) year work experience as a professional nurse within past five (5) years in a hospital, home care, nursing home, oncology, or medical/surgical setting.
Credentialing/Licensing: Must possess current professional license as a Registered Nurse with New York State and be in good standing. Proof of license will be required per New York State Department of Health.
Driving Ability: Hospice of CNY is a home care health care employer and requires travel. All employees must have a valid driver’s license and possess reliable transportation. Our geographic area of coverage encompasses Onondaga, Madison, Oswego, and Cayuga counties.
- Must have strong clinical and communication skills.
- Is a member of an interdisciplinary team, communicating patient status and changes to the team.
- Is responsible for documentation of all care and for the administration of medication.
- Acts as liaisons to other caregivers and spiritual care persons.
- Provides comprehensive care for patients.
- Has many roles, including providing critical hands-on care to patients.
This is a full-time salaried position scheduled from Monday through Friday with eligibility for insurance and benefits. This includes, but is not limited to medical, dental, and various supplemental insurance options, eligibility to participate in the company’s retirement plan fund, and paid time off.
Job Type: Full-time; 40 hours per week
Salary Range: $38.00 – $43.00 per hour
