Intake Coordinator – Hospice & Palliative Care (Rural Communities Focus)
Location: Wailuku, Maui (with service support for Molokaʻi, Hāna, and Lānaʻi)

Position Summary
Hospice Maui is seeking a compassionate and detail-oriented Intake Coordinator to serve as one of the first points of contact for patients, families, physicians, and referral partners. This role plays a vital part in coordinating timely hospice and palliative care admissions, with primary support for our rural neighbor island and East Maui communities of Molokaʻi, Hāna, and Lānaʻi, as well as our Palliative Care program.
The Intake Coordinator gathers essential patient information, facilitates referrals, supports admission workflows, and ensures warm, respectful communication with individuals during vulnerable moments.
Key Responsibilities
- Serve as the primary contact for hospice and palliative care referrals and inquiries
- Support intake coordination for patients in Molokaʻi, Hāna, Lānaʻi, and Maui
- Conduct initial interviews with patients, families, caregivers, and referral sources
- Collect and enter demographic, clinical, and insurance information accurately
- Process admission paperwork and coordinate clinical evaluation visits
- Provide education regarding hospice and palliative care services and eligibility
- Respond to phone calls and visitors with professionalism, warmth, and empathy
- Maintain confidentiality and compliance with all regulatory requirements
- Collaborate closely with the interdisciplinary care team
- Perform other related duties as assigned
Qualifications
- High School Diploma required; Associate’s Degree preferred
- Minimum 3 years office and administrative experience required
- Proficiency with Microsoft Office, Google Workspace, smartphones, and data base entry
- Strong communication, organizational, and customer service skills
- Ability to work with sensitivity and compassion with seriously ill patients and families
- Excellent attention to detail and ability to manage multiple priorities
- Cultural awareness and respect for the diverse communities we serve
Physical Requirements
- Ability to sit, stand, walk, and use hands for office and computer work
- Occasional lifting up to 30 pounds
- Ability to communicate clearly by phone and in person
