We are looking to hire a full-time Registered Nurse Case Manager to assist with our hospice patient visits in the Golden Triangle! The RNCM will work with a team of healthcare providers to ensure our home health patients are receiving quality care.
Responsibilities:
- Conducting routine visits, admissions visits and/or follow-up visits at home or other facilities settings.
- Coordinating, assessing, and implementing patients' care.
- Providing accurate and timely documentation of patient services to reflect plans of care.
- Flexibility to be available days, evenings and weekends.
