What are the responsibilities and job description for the QAPI (Quality Assurance Performance Improvement) and HOPE Assessment Coordinator position at Hospice Care Plus?
JOB SUMMARY:
The QAPI Coordinator is responsible for operating and monitoring the QAPI program, ensuring the clinical practices of the organization follow all federal and state compliance through regulatory guidance and ensuring quality of care through program integrity of Hospice Care Plus. The QAPI Coordinator works collaboratively with the Director of Clinical Services Care and the CEO to ensure compliance of all hospice programs through Quality Assurance and Performance Improvement. This position also is responsible for review and submission of CMS required HOPE (Hospice Outcome Patient Evaluation) assessments.

EDUCATION AND QUALIFICATIONS:
- RN from an accredited school of nursing (Bachelors preferred).
- 3 years of nursing experience with direct experience in a health care setting and compliance
- Proficient in Electronic Medical Record usage and tracking and trending of healthcare data.
- Assists in the planning, implementation, operation, and evaluation of Hospice care to terminally ill patients and their families through QAPI review.
- Monitors all agency activities for compliance with Medicare and Medicaid requirements and ensure policies and procedures are in place.
- Responsible for the overall development, implementation, and evaluation of Hospice Care Plus clinical programs that meet federal and state guidelines as part of the QAPI program in collaboration with the Director of Clinical Services, CEO and Medical Director.
- Develop a Quality Assurance and Performance Improvement Plan yearly.
- Proactive review of CAHPS survey data and Customer Service Program as part of QAPI.
- HOPE CMS submission and oversight
