Job Description

- Assess and evaluate patient needs/problems
- Identify mutually agreed upon goals for care plan with patients
- Report patient status, updates and discharge planning process to clinical leadership, attending physician and hospice physician
Responsibilities include:
- Perform an annual on-site visit to the location where a patient is receiving care
- Attend staff meetings and educational in-services as needed
- Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility)
Requirements
- Current unencumbered State Professional Nurse License
- Valid drivers license and good driving record
- Ability to work in a field setting and make sound nursing judgment
Salary : $80,000 - $110,000
