The Opportunity: We are looking for a highly qualified and career-minded Licensed Practical Nurse LPN to perform telehealth installation in patient homes and assist RN in monitoring of vital signs. You will work closely with an interdisciplinary team of nursing and therapists as well as patients and their families. Shift: Hours are M-Th 8:00- 4:30pm and rotating weekends. The first week is M-Th plus Saturday. The next week is M-Th plus Sunday. The weekdays involve installing and performing in-office duties and every other weekend to monitor vital signs remotely from home. M-Th Installing telemonitors in patient homes, providing education to patients on use. Pickup, cleaning, troubleshooting, and inventory of telemonitoring units. Scheduling visits with patients. Every other weekend: monitoring patient VS from home, calling patients to assess for symptoms, contacting MD office for any concerns when applicable. The Work: Installation and pickup of telehealth monitors in patient homes Monitoring of biometrics every other weekend and coverage for high volume, vacation times Educating patients of use of equipment, disease processes, and medications Assist with troubleshooting telehealth monitors Clean, restock, and keep track of inventory Chart in medical records and HRS program when indicated Contact physicians for patient concerns, vital signs outside of parameters and receive and write any new orders received from physicians Assign visits in EPIC scheduling as indicated Why Saint Luke’s?
