This position coordinates admissions, discharges, follow-up care, and operational support for the hospitalist program. The nurse facilitates Chronic Care Management (CCM) and Transitional Care Management (TCM), performs clinic duties as needed, and focuses on accurate documentation, billing coordination between hospital and clinic, and program support to improve patient outcomes.Essential Duties & ResponsibilitiesCare Transitions & CoordinationPatient Admission Coordination: Manage the admission process for clinic patients requiring hospital care, ensuring all required documentation and pre-admission steps are completed efficiently.Discharge Planning: Coordinate discharges, ensuring instructions are clearly communicated to patients and clinic staff and that resources are in place for a safe transition to home or back to clinic care.Follow-Up Care: Schedule and manage post-discharge and chronic care follow-up visits. Ensure patients understand their care plans and that care is coordinated across providers.Chronic & Transitional Care ManagementTransitional Care Management (TCM):Contact patients within required post-discharge timeframes.Schedule and coordinate follow-up appointments.Collaborate with hospitalists and clinic providers to ensure continuity of care.Chronic Care Management (CCM):Assist in creating and maintaining care plans for patients with chronic conditions.Complete regular check-ins and coordinate services across providers.Support documentation and billing requirements for CCM services.Documentation, Billing, & Program SupportBilling and Documentation:Oversee transfer of billing information between the hospital and clinic, helping resolve discrepancies.Ensure complete, accurate, and timely documentation of patient care and transition events.Hospitalist Program Support:Serve as a central point of contact for the hospitalist team, patients, families, and clinic staff.Support daily hospitalist program workflows to promote optimal patient outcomes.Communication:Act as the primary liaison between hospitalist providers and clinic teams, ensuring clear, consistent communication of care plans and patient needs.Quality, Compliance, & Other DutiesQuality Improvement: Identify opportunities to strengthen care transitions, reduce readmissions, and improve CCM/TCM effectiveness. Help implement best practices.Compliance: Carry out work in accordance with clinic/hospital policies, HIPAA, and applicable healthcare regulations.Perform other related duties as assigned.Supervisory ResponsibilitiesThis position does not supervise other employees.Education, Certifications & LicensesRequired:Current Registered Nurse (RN) license (Wyoming or compact state).Basic Life Support (BLS) certification.Preferred:Additional certification or training in Chronic Care Management (CCM) or Transitional Care Management (TCM).ExperienceRequired:Minimum of 2 years of nursing experience, preferably in a hospitalist, care coordination, or clinic setting.Preferred:Experience with admissions, discharges, and transitional care workflows.Familiarity with medical billing, coding support, and documentation standards related to TCM/CCM.Required Skills & AbilitiesClinical Knowledge: Strong understanding of hospital and outpatient clinic operations, especially admissions, discharges, and care coordination.Communication: Excellent verbal and written communication skills with patients, families, and multidisciplinary care teams.Organizational Skills: High-level multitasking and time-management ability; capable of handling multiple priorities simultaneously.Problem-Solving: Proactive in identifying and resolving barriers during transitions and chronic care follow-up.Technical Skills: Comfortable using EHR systems, billing platforms, and office software.Empathy: Compassionate, patient-centered approach.Teamwork: Collaborative mindset and ability to work smoothly with hospitalists, clinic staff, and external care partners.Working ConditionsPrimarily clinic-based with frequent coordination with hospital partners.Regular interaction with patients, families, hospitalist providers, and clinic care teams.Work may include phone-based follow-ups and care coordination tasks.Occasional schedule flexibility may be needed to meet transition-of-care timelines.Equal Opportunity EmployerHoskinson Health & Wellness Clinic is an Equal Opportunity Employer.
