- Coordinate and complete accurate MDS assessments, including RAPs and Triggers, in a timely manner
- Review, update, and maintain MDS-related policies, procedures, and resident care plans
- Ensure compliance with all state and federal regulations and facility standards
- Assess resident needs and collaborate with interdisciplinary teams to optimize care outcomes
- Partner with the Director of Nursing (DON) and care team on admissions, discharges, and resident updates
- Serve as a resource and provide guidance to nursing staff on MDS and PDPM best practices
- Participate in audits, surveys, and performance improvement initiatives
- Active Registered Nurse (RN) license in Pennsylvania (required)
- 3–5 years of RNAC or MDS experience in a skilled nursing or long-term care setting
- Strong knowledge of PDPM, MDS, and managed care systems
- Excellent organizational, leadership, and communication skills
- Experience with EMR systems and regulatory documentation
- Ability to train, mentor, and support nursing staff
