Nurse Practitioner – Horizons Healthcare
We are seeking a dedicated Nurse Practitioner (NP) to join our growing team. The ideal candidate will provide high-quality medical care, support our mission of excellence, and work collaboratively with our Medical Director and staff to ensure the best outcomes for our patients.
Key Responsibilities
- Conduct comprehensive patient assessments, including medical histories and physical exams
- Diagnose and manage acute and chronic conditions within scope of practice
- Provide specialized care in women’s health, hormone replacement therapy, and weight management
- Develop and implement individualized treatment plans
- Prescribe medications and order diagnostic tests as appropriate
- Educate patients on preventive care, lifestyle changes, and treatment options
- Collaborate with physicians and multidisciplinary team members to optimize patient care
- Maintain accurate and timely medical records

- Current Nurse Practitioner license (State of Ohio
- New Grads are welcome to apply.
-2 years of clinical experience (experience in women's health preferred)
- Strong communication and interpersonal skills
