Position: Shift Supervisor Professional
Reports to: Facility Manager
Supervises: Direct Care Professions,Substitutes
Qualifications: One must have the ability to work with an interdisciplinary team, possess good interpersonal and organizational skills. Previous supervisory experience is preferred. Must have the ability to define problems, collect data, establish facts and draw valid conclusions: write letters, paper and reports: handle sensitive face-to face contacts and maintain a friendly atmosphere. Must have a valid driver's license. will be required to transport individuals for activities, doctor's appointments, etc., in an agency vehicle.
Basic Function: The Shift Supervisor Professional will provide a home environment for the individuals residing in the home. The Shift Supervisor Professional will work directly with the individuals, individually and in group, to help meet their various social, Physical and Psychological needs.
Primary Responsibilities: 1. Supervise directly all activities of the home.
2. Implement regulations and programs set forth by Horizons of Tuscarawas and Carroll Counties, Inc.
3. Provide the supervision necessary to carry out the operation and maintenance of the home, including menu planning, food preparation, food purchasing, visitation, preparation of reports, individual service plan implementation, and reporting any concerns of the staff for operation of the home to the Program Manager, Area Coordinator or Program Coordinator.
4. Maintain a clean, healthful and presentable physical environment for the individuals.
5. Follow the fire and safety regulations and carry out required fire drills to assure the safety of all individuals.
6. Record each individual's progress on Documentation Sheets, as well as assuring the confidentiality of all records keep within the home.
7. Attend all staff meetings. Ensure that notes are taken . Turn in to office.
8. Assure periodic checkups and provide transportation for individuals to medical, dental, counseling appointments, and extra-curricular activities.
9. Perform household maintenance (special occasions for some homes) and inform the Maintenance Engineer of repair work that is needed on the house, equipment, vehicles or grounds, unless there is a landlord.
10. Oversee individuals: instruct and assist individuals in housekeeping, meal preparation, personal hygiene, personal grooming, recreation skills, social skills and other daily living skills that are designated by their IPS.
