What are the responsibilities and job description for the Registered Nurse RN Case Manager position at Horizon Hospice & Palliative Care?
JOB DESCRIPTION:
As a member of the Interdisciplinary Team, the Case Manager Nurse shall be responsible to the Director of Clinical Services and works under the general direction of the attending Physician, in provision of the comfort and health of patients and families.

DUTIES:
1. Performs focused patient assessments that include the patient’s physical, psychosocial, emotional, and environmental needs, including functional status, ADLs and fall risk.
2. Collaborate with the patient and patient’s care team in developing and implementing the plan of care, making necessary revisions as needed.
3. Act as patient advocate with the attending physician and other care providers involved in the patient’s care to manage both physical and/or emotional discomfort and provide symptom relief.
4. Perform ongoing medication reconciliations as well as provide continual medication education to family/patient regarding administration, side effects, safety and disposal of medications. Collaborate with patient/family and providers in deprescribing for patient comfort as needed.
5. Initiates preventative and rehabilitative nursing procedures as appropriate for the patient’s care and safety.
6. Perform post-mortem visits and pronouncement of patient death. Nurse will allow family time to grieve and provide support as needed, to include post mortem care as desired by family. Nurse will prepare documentation and perform required phone calls and post-mortem activities within a timely manner.
7. Prepare clinical notes that demonstrate progress toward established goals, in a timely manner as set forth by nursing manager.
8. Exchanges information with other team members as appropriate. This includes discussion with the Hospice Home Aide (HHA) and Licensed Professional Nurse (LPN) regarding their assignments and care plans.
9. Performs an indirect HHA supervision, at most, every 14 days when aide, and a direct supervision twice per year.
10. Performs an LPN supervision within 14 days after their visit is performed, assessing patient care given and documentation.
11. Provide patient and family/caregiver education regarding the disease process, self-care techniques, and processes for addressing ethical issues.
12. Supervises and assists in teaching other nursing personnel as needed.
13. Cooperatively works with the Social Worker in making appropriate community referrals.
14. Adhere to all of HHPC policy and procedures to include state guidelines on Infection Control and PPE requirements. Maintain required testing and training related to these guidelines.
15. Participates in on-call responsibilities.
16. Participates in coverage for vacation and absences as needed.
17. Participates in required in-services and meetings as set forth by nursing supervisors or department heads.
18. Seeks assistance from supervisor when assigned nursing procedures that are beyond their training.
19. Perform all tasks assigned by Director of Clinical Services or designee.
WORKING CONDITION AND HAZARDS:
Work includes driving personal vehicle to patients’ home and visiting or attending patients in various degrees of cleanliness and physical comfort. Varying types of weather and road conditions are encountered, along with working/driving during hours of darkness. Employee is exposed to persons in physical distress, including pain and disfigurement. Physical contact with patients may include exposure to body fluids and odors. Direct care contact with patients places employee in “high-risk” category for exposure to blood-borne pathogens. Difficult personalities, depressed and anxious individuals, stressful situations and intense grief are frequently encountered. Working hours are set by nursing manager but may be extended depending on daily workload/patient needs PRN. Visiting schedule set by individual patient/family needs and schedule.
QUALIFICATIONS:
• Current RN license in the State of Washington
• Two years clinical/home care experience preferred
• Skilled in nursing practice, communication and commitment to Hospice Philosophy
• Must have approved Criminal Background check
• Must successfully complete performance evaluation and a nursing skills competency evaluation during probationary period and annually thereafter.
• Valid Driver’s License and active Auto Insurance Card.
Salary : $100,000 - $115,000
