What are the responsibilities and job description for the Aide position at Horizon Hospice & Palliative Care?
JOB DESCRIPTION:
As a member of the Interdisciplinary Team the Hospice Aide shall be responsible to the Clinical Supervisor or designated RN for providing personal care/services to the terminally ill patient and performance of related tasks in the patient/family home under the supervision of a Registered Nurse and in accordance with the plan of care.
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
1. Provide or assist with bathing and personal hygiene needs, as noted in the care plan, under RN direction.
2. Assist patient in and out of bed/chair.
3. Perform simple, non-skilled clinical procedures under RN direction.
4. Report changes in patient/family condition and needs.
5. Provide respite care while primary care giver is absent.
6. Perform light housekeeping duties (vacuuming, sweeping, dusting, emptying garbage, and wash dishes).
7. Change linen, assist with laundry.
8. Clean up bathroom after cares provided.
9. Run short errands and prepare meals.
10. Be responsible for patients supply maintenance, ordering and delivering as needed.
11. Maintain accurate, up-to-date patient records.
12. Attend weekly IDT meetings when requested.
13. Participate in Horizon Hospice orientation and continued education.
WORKING CONDITIONS AND HAZARDS:
Work includes driving personal vehicles to patient’s home, throughout the county and visiting or attending patients in various degrees of cleanliness and physical comfort. Varying types of weather and road conditions are encountered. Employees are exposed to people in physical distress including pain and disfigurement. Physical contact with patients is possible and may include exposure to body fluids and odors. Direct care contact with patient’s places employee in “high-risk” category for exposure to blood borne pathogens. Difficult personalities, depressed and anxious individuals, stressful situations and intense grief are frequently encountered. Working hours are determined by individual contract and daily schedule is set up with patient/family/CG.
Aide/Homemaker role requires lifting, bending, stooping and standing for specific periods of time.
QUALIFICATIONS:
Current certification to practice as a Nursing Assistant in the State of Washington.
Food Handler’s permit.
CPR certification.
Ability to take directions.
Current driver’s license and reliable car.
Must have approved Criminal Background Check.
Will be required to have P.A.T & Kraus-Weber completed.
Salary : $22 - $24
