Role Description
This is a full-time on-site role for an RN Case Manager - Hospice based in Arvada, CO. The RN Case Manager will coordinate and oversee the care of hospice patients, including assessing patient needs, developing and implementing individualized care plans, and providing nursing care directly. The role also involves educating patients and families, coordinating with the interdisciplinary team, managing pain and symptom control, and facilitating communication with other healthcare providers and support services. The RN Case Manager plays a critical role in ensuring patient comfort and quality of care.
Qualifications
- Strong nursing and medical care skills, with an active RN license in Colorado and a solid foundation in clinical practices
- Experience in Case Management, including the ability to create and oversee individualized care plans
- Expertise in Discharge Planning and Utilization Management to ensure smooth transitions and optimal use of resources
- Compassionate communication skills for effectively addressing patient and family needs
- Ability to work collaboratively within an interdisciplinary team to ensure the best outcomes for patients
- Hospice or palliative care experience of at least one year is highly preferred
- A valid driver’s license and reliable transportation for in-home patient visits
