Start of Care Physical Therapist | Caldwell, ID | Part-Time | Horizon Home Health and Hospice

Start of Care Physical Therapist Horizon Home Health & Hospice is seeking a part‑time, non‑exempt, per‑visit Start of Care (SOC) Physical Therapist to join our home‑health team in Caldwell, Idaho. This role is ideal for PTs who excel in first‑visit evaluations, functional mobility assessments, and establishing a strong therapeutic foundation for each patient’s episode of care.

As a SOC PT, you will complete comprehensive admission assessments, initiate individualized treatment plans, and collaborate closely with our interdisciplinary team to ensure timely, accurate, and patient‑centered service initiation.

SOC PT Responsibilities

• Complete Start‑of‑Care physical therapy evaluations in patient homes.

• Develop individualized treatment plans focused on functional mobility, safety, and independence.

• Conduct comprehensive assessments including gait analysis, strength testing, balance evaluation, and home‑safety review.

• Establish strong patient rapport and set expectations for the home‑health episode.

• Collaborate with interdisciplinary team members to ensure timely initiation of services.

• Provide therapeutic interventions as needed during SOC visits.

• Recommend and educate on durable medical equipment and home‑safety modifications.

• Maintain accurate, timely documentation in accordance with regulatory and organizational standards.

• Communicate effectively with physicians, caregivers, and internal team members.

PT Requirements

• Current Idaho Physical Therapist license (or ability to obtain).

• One year of PT experience preferred; home‑health or SOC experience a plus.

• Strong communication, assessment, and problem‑solving skills.

• Ability to work independently in a field‑based environment.

• Reliable transportation and valid driver’s license.
