Horizon Health Services is seeking a compassionate and dependable Registered Nurse (RN) to perform nurse monitoring visits for clients in the Sudlersville, Maryland area. This flexible, per-visit position is ideal for an RN who enjoys working independently while providing high-quality care in clients' homes.
Responsibilities
- Conduct scheduled nurse monitoring visits in clients' homes.
- Assess clients' health status and monitor overall well-being.
- Evaluate medication compliance and identify any health or safety concerns.
- Review and update individualized plans of care as needed.
- Educate clients and caregivers on health management and safety.
- Communicate changes in client condition to the appropriate healthcare team.
- Complete accurate and timely documentation in accordance with agency and Maryland regulations.
- Ensure compliance with all agency policies and standards of care.
Qualifications
- Active Maryland Registered Nurse (RN) license.
- Current CPR certification.
- Home health or community nursing experience preferred.
- Strong assessment, documentation, and communication skills.
- Reliable transportation and a valid driver's license.
- Ability to work independently and manage a flexible schedule.
