- Licensed Practical Nurse (LPN) for the Hospice and Home Care divisions of Horizon Health, Inc.
- Participate in coordination of services, including intake of referrals, medication orders, authorizations, and client record review.
- Communicates with clients and their families to ensure person-centered care.
- Hospice and Home Care visits as needed.
- Meetings as required.
