Licensed Practical Nurse (LPN) – Assisted Living

About the Role

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Assisted Living team. In this role, you will play a vital part in ensuring the safe, high-quality care and well-being of our residents, while promoting independence, dignity, and quality of life.

This position offers a blend of clinical care, leadership, and hands-on support, making it ideal for an LPN who enjoys meaningful resident relationships and a team-oriented environment.

Key Responsibilities

Resident Care & Clinical Duties

• Provide direct resident care in accordance with individualized care plans
• Administer medications and injections safely and accurately
• Monitor resident health, vital signs, and overall well-being
• Observe, document, and report changes in condition
• Follow physician orders and ensure proper implementation
• Respond effectively to emergencies and changing resident needs

Leadership & Coordination

• Supervise care staff and ensure adequate staffing coverage
• Communicate with physicians, families, social workers, and other care partners
• Coordinate and follow through on resident care needs
• Assist with training and mentoring staff

Daily Living Support

• Assist residents with personal care tasks including bathing, grooming, toileting, and mobility
• Promote independence while ensuring safety and dignity
• Support nutrition, hydration, and social engagement

Environment & Safety

• Maintain a clean, safe, and welcoming environment
• Follow infection control procedures and safety protocols
• Report hazards, incidents, and equipment issues promptly
• Ensure compliance with all HIPAA and regulatory requirements

Additional Duties

• Assist with household tasks such as meal prep, light housekeeping, or transportation (as assigned)
• Attend required trainings and staff meetings
• Perform additional duties as assigned by leadership

Qualifications Required

• Current Licensed Practical Nurse (LPN) license
• Strong clinical and resident care skills
• Ability to communicate effectively with residents, families, and team members
• Knowledge of medication administration and basic nursing procedures
• Ability to work flexible hours, including weekends and holidays

Preferred

• Experience in assisted living, long-term care, or home care
• Leadership or supervisory experience
• Familiarity with aging-related conditions and behaviors

Pay: $25.30 - $31.25 per hour
