Licensed Practical Nurse (LPN) – Assisted Living
Location: Harmony House of Brainerd

Reports To: Assisted Living Director
About the Role
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Assisted Living team. In this role, you will play a vital part in ensuring the safe, high-quality care and well-being of our residents, while promoting independence, dignity, and quality of life.
This position offers a blend of clinical care, leadership, and hands-on support, making it ideal for an LPN who enjoys meaningful resident relationships and a team-oriented environment.
Key Responsibilities
Resident Care & Clinical Duties
- Provide direct resident care in accordance with individualized care plans
- Administer medications and injections safely and accurately
- Monitor resident health, vital signs, and overall well-being
- Observe, document, and report changes in condition
- Follow physician orders and ensure proper implementation
- Respond effectively to emergencies and changing resident needs
Leadership & Coordination
- Supervise care staff and ensure adequate staffing coverage
- Communicate with physicians, families, social workers, and other care partners
- Coordinate and follow through on resident care needs
- Assist with training and mentoring staff
Daily Living Support
- Assist residents with personal care tasks including bathing, grooming, toileting, and mobility
- Promote independence while ensuring safety and dignity
- Support nutrition, hydration, and social engagement
Environment & Safety
- Maintain a clean, safe, and welcoming environment
- Follow infection control procedures and safety protocols
- Report hazards, incidents, and equipment issues promptly
- Ensure compliance with all HIPAA and regulatory requirements
Additional Duties
- Assist with household tasks such as meal prep, light housekeeping, or transportation (as assigned)
- Attend required trainings and staff meetings
- Perform additional duties as assigned by leadership
Qualifications Required
- Current Licensed Practical Nurse (LPN) license
- Strong clinical and resident care skills
- Ability to communicate effectively with residents, families, and team members
- Knowledge of medication administration and basic nursing procedures
- Ability to work flexible hours, including weekends and holidays
Preferred
- Experience in assisted living, long-term care, or home care
- Leadership or supervisory experience
- Familiarity with aging-related conditions and behaviors

Pay: $25.30 - $31.25 per hour
