Housing Specialist

The Housing Specialist provides in-home and community-based support to individuals working to obtain and maintain independent housing. This role focuses on building independent living skills, supporting housing stability, and delivering trauma-informed, recovery-oriented services. The Housing Specialist manages an assigned caseload, completes required documentation, and promotes a positive experience where Kindness Matters.

This is a safety-sensitive position and may include driving and other duties that impact the safety and well-being of clients and staff.

Responsibilities:

• Provide in-home and community support services in accordance with individualized treatment plans.
• Assist individuals with housing searches, applications, and placement.
• Teach and support independent living skills, including personal care, budgeting, meal preparation, and household management.
• Connect individuals to community resources and support community integration.
• Coordinate care with outreach teams and participate in LCEH meetings.
• Complete clinical and billing documentation daily.
• Provide medication education and transportation to appointments as needed
• Perform other duties as assigned

Qualifications Required:

• HS diploma or GED; plus 4 years’ experience in behavioral health or combined behavioral health and education and experience with at least 1 year case management.
• Must be 21 years of age or older.
• Possess valid driver’s license with acceptable driving record, home telephone, and reliable transportation.
• Certification in first aid, CPR, and Crisis De-escalation training such as CPI (agency will provide training if candidate does not have current certification).
• Ability to lift 50 pounds.
• Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
• Must obtain and maintain a fingerprint clearance card and pass a background check.

Mental and Emotional

• Demonstrate good written and verbal communication skills.
• Able to positively interact and develop rapport with mentally ill participants and their families, professional and support staff and various levels of staff from community agencies.
• Able to maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations.

Physical

• Long periods of sitting, ability to bend, lift and carry up to 50 pounds.
• Able to operate phone, Fax, copier, and a computer.
• Must be fully ambulatory and able to assist participants.
• Must be able to operate an Agency vehicle.

Qualifications Preferred:

• Experience working in Integrated Healthcare, Behavioral Health, Residential or Housing.
• Advanced knowledge of cultural diversity and sensitivity.
• Bilingual – English/Spanish
