Horizon Adult Health Care in Pineville, KY, an EOE, has the following job opportunity available: Patient Care Coordinator
The position is full time, daytime hours, Monday-Friday. The position will require public education about services offered and assistance with applying for services. Experience working with the geriatric/disabled population preferred but not required. The position will require travel within Bell and Harlan Counties. We are looking for an energetic, positive attitude, and smiling face. Competitive pay and full benefit package available including medical, dental, vision, 401k, holiday pay, paid days off and more. During extenuating circumstances such as pandemics, job duties may transition to include providing additional services in client homes.
