Position Summary
HopeSparks is a contracted partner of Elevate Health to provide Community-Based Care Coordination 10/1/2024-12/31/2026. The HopeSparks Family Support Specialist assigned to this contract will provide community-based care coordination services as defined by the Elevate Health Community Care Hub contract with HopeSparks. Elevate Health is a Pierce County-based non-profit collaborative serving as a backbone organization to a cross-sector network dedicated to improving the health and well-being of all Pierce County residents through collective action.
The goal of Community Based Care Coordination services are to support Pierce County residents' access to non-clinical community-based care coordination. The Family Support Specialists assigned to this work are defined by Elevate Health as frontline public health workers who are trusted members of and/or have unique understanding of the experience, language, and culture of a particular population. These services are provided in the family's home, community, and/or via telehealth (depending on the client's need and contract requirements) to support and build on family strengths.

Essential Duties & Responsibilities
- Must be able to engage children, youth, and families in a full range of family support services either in-person or via telehealth (through video and/or phone conferencing), including:
- in-home and/or remote case management services to families in order to build skills in parenting, child development, health, promotion, and violence prevention
- collaboration on a multi-agency, multi-disciplinary team in a manner that promotes mutual respect, open communication, and joint problem solving and which values and builds the unique strengths of all service providers
- organizing and facilitating parent support/education groups and community development activities as appropriate/needed
- Enter participant/client services and documentation within the technology platform chosen and provided by Elevate Health and submit reports to the Director of Family Support Services on a regular basis in a timely manner, per contract deliverables
- Make a minimum of one in-person visit each month with participants/clients, to the extent that safety allows.
- Remain knowledgeable of all expectations and procedures associated with funded work and its documentation on the care coordination platform
- HopeSparks will provide supervision for the Family Support Specialist that will include, but not limited to, case consultation, quality assurances, ongoing training, and liaison between HopeSparks and Elevate Health.
- Attend a minimum of one (1) Community-Based Worker Forum meeting per quarter.
- Complete quarterly surveys as needed to provide Elevate Health with feedback regarding program operations and efficacy.
Qualifications
- Possess a minimum of a Bachelor's Degree in one of the human services fields OR year-for-year direct experience in a like position.
- Possess a valid driver's license and use an automobile for home visits and local travel.
- Willingness to participate and provide all necessary documentation for both State Patrol and FBI checks; may be required to share documentation with Contract partner agencies.

Job Conditions
- Work may be performed in a client's home, an agency office setting, or community partner location.
- Ability to stand, sit, talk, and listen/hear for extended periods (8 hours) daily in order to perform administrative duties
- Ability to use hands to finger, handle or write for extended periods (8 hours) daily
- Ability to reach with hands and arms, and climb or balance to perform filing duties for extended periods (6-8 hours) daily
- Ability to ascend and descend stairs to access clients' homes
- Ability to lift items weighing up to 50 pounds for filing duties
- Ability to focus and see items with close, distance, color and peripheral vision, and depth perception to drive, perform computer work, and file (8 hours)
