Licensed Vocational Nurse (LVN)

The Hospice Licensed Vocational Nurse (LVN) will work under the supervision of a Registered Nurse Case Manager and provide skilled patient care as outlined in the written plan of care.

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Schedule:

Monday - Friday; 8:30am - 5:00pm

Location:

Contra Costa County and some areas of Alameda County

Essential Duties & Responsibilities include (but are not limited to):

• Assist a primary nurse in providing direct care.
• Initiate communication with physicians and team leaders, IPG members, HHA’s and other health care providers as needed to facilitate optimal care.
• Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Team Leader. Note: The LVN receives direction from the RN on-site and may not alter the patient/family plan of care.
• Attend patient conference and participate in discussion of issues to facilitate optimal care.

Education/Experience:

• Completion of a Licensed Vocational Nursing Education Program from an accredited school.
• Minimum of one year of LVN experience preferably in an acute care setting. Experience in a home health/ hospice setting preferred but not required.

Certificates, Licenses, Registrations:

• Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.
• Valid California certification in Intravenous Therapy & Blood Withdrawal preferred

Pay Range:
$38.65 - $48.40 hourly
