Job Overview
Hopeful Hands LLC is seeking a knowledgeable and client-centered Case Manager to provide behavioral health provider case management services in accordance with AHCCCS AMPM Policy 570. This role focuses on supporting adults with behavioral health and substance use needs through coordination of care, linkage to services, and ongoing monitoring to improve treatment outcomes.
The Case Manager will maintain regular contact with members, ensure continuity of care across services, and support clients in achieving individualized treatment goals.
Key Responsibilities
- Conduct intake assessments addressing behavioral health history, social needs, housing, physical health, and other SDOH
- Develop, implement, and update individualized service plans based on member goals and clinical needs
- Provide routine case management contact (2–3 times weekly or as clinically indicated), monitoring progress and adjusting services when needed
- Identify, coordinate, and link members to behavioral health, medical, housing, vocational, educational, and community supports
- Assist members in accessing AHCCCS-covered services, including therapy, PCP services, medications, transportation, and supportive programs
- Coordinate care across multiple providers, including therapists, PCPs, crisis teams, housing providers, probation/parole, schools, and natural supports
- Participate in treatment team meetings, case staffings, and interagency coordination
- Provide psychoeducation related to diagnosis, treatment, safety planning, and crisis supports
- Conduct outreach and follow-up after missed appointments or crisis events; complete required follow-up within 72 hours post-crisis
- Assist with AHCCCS, DES, Social Security/benefits applications (SOAR activities only if SOAR-certified)
- Ensure crisis and safety plans are updated and shared appropriately
- Document all services in compliance with AHCCCS, AMPM 570, HIPAA, and agency standards
- Maintain accessibility for members and coordinate with backup case managers when unavailable
- Collaborate with Hopeful Hands’ clinical team to ensure appropriate supervision and quality of care
Qualifications
- Must meet Arizona Department of Health Services personnel member requirements of: at least 21 years old; or at least 18 year of age and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice.
- 1–2 years of behavioral health, case management, or community support experience preferred
- Knowledge of AHCCCS requirements, AMPM 570 provider case management expectations, and community resources
- Strong skills in communication, organization, coordination, and documentation
- Ability to manage caseloads ethically and maintain professional boundaries
- Valid driver’s license and reliable transportation
- Current fingerprint clearance card and CPR/First Aid, or ability to obtain
- SOAR certification optional (but required if performing SOAR-specific benefit tasks)
