Position Summary
HopeBridge Recovery is seeking a compassionate and motivated Case Manager to support clients throughout their recovery journey. The Case Manager plays a vital role in coordinating care, assisting with discharge planning, and connecting clients to essential community resources that promote long-term stability and independence. This position serves as a bridge between clients and service providers, ensuring continuity of care and a seamless treatment experience.
Core Responsibilities
Assessment and Care Planning
- Conduct comprehensive assessments to identify clients’ clinical, social, vocational, and housing needs.
- Participate in the development of the individualized treatment, recovery, and discharge plans in collaboration with the clinical team and the client.
Care Coordination
- Coordinate services among internal and external providers to ensure continuity of care and smooth transitions between levels of treatment.
- Assist clients with connecting to next levels of care, outpatient services, and ongoing support programs.
Discharge Planning & Continuity of Care
- Support clients with discharge planning, ensuring follow-up appointments, services, and resources are in place.
- Ensure continuation of services post-discharge to reduce relapse risk and promote stability.
Resource Navigation & Advocacy
- Assist clients with job searches, resume support, and employment resources.
- Support applications for benefits such as SNAP, SSDI, Medicaid, and other public assistance programs.
- Assist clients with obtaining or applying for housing and other basic needs.
- Advocate on behalf of clients to access appropriate community resources.
Progress Monitoring
- Monitor client progress toward goals and adjust care plans as needed in collaboration with the treatment team.
- Provide ongoing support to help clients sustain independence and recovery.
Documentation and Compliance
- Maintain accurate, timely, and confidential documentation in compliance with program policies, state regulations, and ethical standards.
Crisis Intervention
- Provide immediate support during client crises and coordinate emergency or higher-level services when necessary.
Psychoeducation and Support
- Educate clients and families on mental health and substance use conditions, coping strategies, and recovery tools.
- Provide guidance and support throughout treatment and discharge planning.
Collaboration
- Participate in multidisciplinary team meetings.
- Communicate and collaborate effectively with clinicians, providers, and community partners.
Qualifications and Skills
Certification/Licensure
- Must hold appropriate certification and/or licensure, (i.e. QMHS, SWA, LSW, LPC, or higher).
Education
- Minimum of a Bachelor’s degree preferred in social work, behavioral health, or a related field.
Experience
- Three (3) years of prior experience in behavioral health, substance use treatment, or social services preferred.
Skills and Competencies
- Strong communication, organizational, and problem-solving skills.
- Knowledge of community resources related to housing, employment, benefits, and healthcare.
- Computer proficiency and ability to maintain accurate electronic records.
- Empathy, professionalism, and ability to maintain confidentiality and appropriate boundaries.
- Ability to work independently and as part of a collaborative team.
