Position Summary:This full-time salaried leadership role is responsible for providing clinical, administrative, and operational supervision to the Care Management Team while promoting whole-person, integrated care.
Requirements
Qualifications:Candidates must be:
- Provisionally or fully licensed as an LCSW, LCMHC, LPA, LMFT, or RN.
- Have at least 3 years of experience providing care management, case management, or care coordination.
- Experience working with individuals with co-occurring Behavioral Health and I/DD needs is preferred.
Key Responsibilities:
- Provide clinical and administrative supervision to a care management team consisting of up to 8 Care Managers and 3 Extenders.
- Lead staff recruitment, onboarding, training, performance management, discipline, and termination processes.
- Ensure quality service delivery and compliance with service definitions through activities such as:
- Facilitating case conferences.
- Conducting record audits.
- Observing team performance.
- Participating in home visits and Child and Family Team (CFT) meetings.
- Ensure timely and accurate documentation of all Care Management activities in accordance with policy and program requirements.
