Duties
- Conduct comprehensive patient assessments to identify medical, social, and emotional needs using medical terminology and EMR (Electronic Medical Records) systems.
- Develop individualized care plans that include discharge planning, utilization review, and coordination of services across hospital departments, treatment centers and nursing homes; other qualified settings.
- Facilitate communication between healthcare providers, social workers, patients, and families to ensure understanding of treatment options, care plans, and community stability goals.
- Manage services processes for outcomes, ensuring compliance with HIPAA regulations and managed care protocols.
- Perform utilization management by reviewing medical necessity for procedures
- Support discharge planning by arranging services, equipment, and supports customized to meet the person's needs and stability.
- Participate in multidisciplinary processes to advocate for appropriate housing and resource allocation within a community based setting, support long-term goals.
Skills
- Proven experience collaborating with government agencies, community organizations, and property owners, preferred.
- Strong knowledge of case management, person-centered service planning, and experience working in a diverse, continuum of services in a fast-paced environment.
- Expertise in medical records keeping with a solid understanding of mandated reporting and HIPAA compliance standards.
- Understanding thorough and comprehensive documentation of service outcomes and admin reports, in compliance with fee-for-service outcomes.
- Ability to evaluate participant's eligibility for resources and accomplish efficient searches for available and suitable housing to meet their long-term community goals.
- Excellent communication skills to effectively coordinate among healthcare teams, patients, and their circles of are while demonstrating empathy and professionalism.
- Ability to navigate complex cases involving their care teams recommendations, behavioral health needs, and supporting a holistic discharge care plan, efficiently.
- Experience with discharge planning, intake procedures, managed care policies, and utilization management strategies, preferred.
