This position offers the opportunity to make a meaningful impact on the lives of those in need. **Responsibilities:** - Conduct diagnostic evaluations to assess patients' needs and develop individualized treatment plans - Provide counseling and therapy sessions to address substance abuse issues - Collaborate with healthcare professionals to coordinate patient care and treatment goals - Assist in discharge planning and aftercare support for patients transitioning out of treatment programs - Maintain accurate and confidential patient records in compliance with HIPAA regulations - Utilize medical terminology and behavioral health knowledge in patient interactions - Implement utilization management strategies to optimize patient service delivery **Requirements:** - Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field, preferred.
