Job Status: Part-Time (Non-Exempt / Hourly)/ 16 -24 hours per week (exact schedule TBD)
Compensation: $60-$75 per hour
Primary Responsibilities include:
- Provide physical examinations including Health history evaluation / assessment / review.
- Provide medical treatment of urges and cravings including the medical treatment of physical health conditions.
- Provide management of medical needs, including medication management and monitoring.
- Assess the need and management of medication assisted treatment for substance.
- Provide care through diagnosis, clinical judgment, and management.
- Complete and obtaining pre-authorizations.
- Makes referrals to outside care/facilities or orders diagnostic/laboratory tests,
- Collects, interprets and documents test results for patients.
- Reviews pre-admission referral reports to assist in determining appropriateness for residential treatment.
- Instructs and counsels patients regarding compliance with prescribed medical or physical regimens, medications, health maintenance, and addiction treatment.
- Work with the medical and counseling staff in providing physical health care, and substance use disorder treatment.
- Provides on call support for after hour situations that may occur.
- Provides educational presentations for staff and patients as needed.
- Maintain documentation as per OASAS standards and Hope House policies and procedures.
- Other duties as assigned.

Education and Training:
- At least one (1) year of education, training, and/or experience as a Family (Community) Nurse Practitioner.
- Experience in the treatment of Substance Use Disorders preferred.
- Must be certified and registered as a nurse practitioner by the NYS Education Department practicing within the scope of, and in accordance with, the terms and conditions of such certifications.
- Familiarity with Medication Assisted Treatment (such as Suboxone or Vivitrol)
- Eligible to prescribe for NYS Medicaid
