What are the responsibilities and job description for the Case Manager position at Hope House Augusta?
Case Manager – Residential Substance Use Treatment
Hope House · Augusta, GA · Full-time, Hourly · Bachelor's required
This role is equal parts advocate, coach, and coordinator.
Hope House is a long-term residential recovery program for women and their children in Augusta. Our Case Managers walk alongside residents as they rebuild their lives — managing the practical (benefits, appointments, housing) and the personal (family reunification, life skills).
This isn't a desk job. You'll be in homes, in courtrooms, on campus, and in the car.
What this job actually looks like:
- Managing a caseload: documentation, service plans, appointments, and benefits applications
- Facilitating in-home family sessions and parenting education (discipline, child development, routines, safe sleep)
- Coaching mothers on parenting techniques during real-world moments — not just in a classroom
- Acting as liaison and advocate with DFCS, courts, and other legal entities
- Supporting residents' transition to independent housing, including transitional apartment visits
You'll be good at this if you:
- Can hold both structure and warmth — accountability and compassion aren't opposites here
- Have real comfort teaching or coaching parenting skills, not just referring someone else to do it
- Communicate clearly in writing — DFCS, courts, and your own team depend on your documentation
- Can manage a caseload without losing track of the human being behind it
- Have personal or professional experience that gives you genuine empathy for what recovery and parenting under pressure look like
Requirements:
- Bachelor's degree in Social Science or related field
- 2 years of case management experience with at-risk individuals
- Experience or training in parenting education, child development, or family support services preferred
- Valid driver's license (you'll transport residents and children in Hope House vehicles)
- Comfortable with routine documentation and appointment coordination
Hope House is an equal opportunity employer. We strongly encourage applications from individuals in recovery and those with lived experience in the communities we serve.
