Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health through observation of vital signs and overall condition, promptly reporting any concerns
- Administer medications and perform catheter care following medical instructions and home healthcare regulations
- Support clients with memory care needs, including dementia and Alzheimer's care, ensuring safety and comfort
- Document patient information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Aid with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and hygienic environment
- Use patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility and transfers
Qualifications
- Prior experience in assisted living, nursing homes, or home healthcare settings preferred
- Knowledge of medical terminology, medication administration, and vital signs monitoring
- Experience working with individuals with developmental disabilities or dementia preferred
- Certification in CPR and First Aid required; Basic Life Support (BLS) certification highly desirable
- Familiarity with HIPAA regulations and maintaining patient confidentiality essential
- Ability to work independently while following care plans and communicating effectively with clients and team members
- Physical ability to perform heavy lifting, assist with patient transfers, and provide personal care assistance
