Job Summary

Hope Creek Nursing and Rehabilitation Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our team in East Moline, Illinois. Responsibilities

• Evaluate and treat patients with a wide range of physical therapy needs, including post-operative rehabilitation, chronic pain management, and restoration of function following illness or injury

• Develop and implement individualized treatment plans in collaboration with patients, families, and other healthcare professionals

• Conduct thorough assessments and assessments of patients' physical therapy needs, including range of motion, strength, flexibility, and motor function

• Utilize evidence-based practice guidelines and treatment protocols to ensure best possible outcomes

• Educate patients and families on proper body mechanics, posture, and safety precautions to promote continued progress and independence

• Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and discharge summaries

• Participate in multidisciplinary team meetings to discuss patient care and develop comprehensive treatment plans
